Here at Dive Rutland your safety is the priority in our scuba diving training classes.
Divers have to accept and understand scuba diving can be a serious situation and can result in injury or death. By following safe diving practices this risk can be greatly minimized.
Below are some of the standard medical statements, liability release forms and diving statements you will need to fill out from time to time. We keep all medical statements completely confidential. No information on them will be disclosed and would only be used by Dive Rutland training staff to ensure diver safety.
The dive shop staff/instructor will let you know which forms you need to fill out for your class or activity.
